The staff of the Hudson Park Rehabilitation and Nursing Center is aware that an admission to a health care center is often unexpected, hurried and emotional. Our professionals are ready to provide you with the information necessary to help make an informed decision about the most appropriate care and services you or your loved one require. A transition into a nursing home can be difficult, which is why our staff is here to help you navigate each step of the way, treating you with the dignity and respect you deserve. We appreciate the trust you place with us, and value the relationships we build with our residents and their families.
Our staff works closely with hospital discharge planners and home care agencies as they assist you through the health care continuum. Prior to admission, our admissions coordinator will evaluate each potential resident’s level of care requirements in order to accommodate placement needs. The admission coordinator will also assist residents and their families with completing the required admission application.
Our staff is familiar with the required paperwork for Medicaid, Medicare and private insurance companies. We are happy to assist you in understanding the process of applying for admission, as well as the payment options for placement in a skilled nursing facility.
Hudson Park’s rehabilitation unit specializes in short-term therapy, and our team works with you to build a unique plan of care to meet your needs and goals.
